This electronic version of the Ontario Drug Benefit Formulary / Comparative Drug Index (Formulary / CDI) provides access to a listed drug products database. At this time, information included in Part III (General Benefits / Limited Use) and in Part IX (Nutrition Products/Diabetic Testing Agents) of the Formulary / CDI can be displayed by selecting one or any combination of the following criteria:

• Coverage Status
• Therapeutic Classification
• Manufacturer
• Generic Name
• Brand Name
• Drug Identification Number/Product Identification Number (DIN/PIN) keyword

Some drug products not listed on the Formulary are reimbursed through the Exceptional Access Program (EAP). Some drug products listed as a Limited Use benefit on the Formulary may be reimbursed through the EAP for other indications. Please see the EAP Reimbursement Criteria.
